The concept of WHO "Health 21", is constructed as a health-political framework concept for the region of Europe. In order to achieve this objectives the WHO recommended the implementation of a new care- qualification - the family health nurse. The involved tasks and responsibilities are health promotion, prevention and rehabilitation with the aid of motivation, consultation and promotion of patients and their relatives. This paper shows findings of a literature study for the implementation of the FHN-concept in several European countries. In addition to that competencies of the nursing staff in Europe are examined, that is employed in primary health care and has competencies which are partially related on elements of the FHN-concept. Databases and the internet were used for the enquiry. A WHO study about the establishment of the FHN in Europe shows high differences in the status in the reported countries. In many cases there seem to exist basic problems at the implementation such as funding and recruitment of the nursing staff. This has been confirmed in the analysis of the project reports from Scotland, Germany and Denmark. Additionally available designs and concepts from Sweden, Finland, the Netherlands and Great Britain have been examined. In many cases the nursing staff in the health care systems of Sweden, Finland and Great Britain has competencies which correspond to elements of the FHN-concept. Overall it is clear, that there are potentials of competencies and forms of organisation of care which may be used in the future for the assurance of health care. -- Keywords: Family Health Nursing, Advanced Nursing Practice, Public Health Nursing, Primary Health Care -- Mit dem Konzept "Gesundheit 21" hat die WHO ein gesundheitspolitisches Rahmenkonzept für die Region Europa erstellt. Um diese Ziele zu erreichen, wurde von der WHO die Einführung einer neuen Pflegequalifizierung empfohlen, der Family Health Nurse. Die damit verbundenen Aufgaben sind Gesundheitsförderung, Prävention und Rehabilitation durch Motivation, Beratung und Unterstützung des Patienten und seiner Angehörigen. Der Beitrag stellt Ergebnisse einer Literaturstudie zur Implementierung des FHN-Konzeptes in verschiedenen Ländern Europas vor. Zusätzlich werden Pflegekonzepte in europäischen Ländern untersucht, die in der primären Gesundheitsversorgung tätig sind und deren Kompetenzen auch präventive und gesundheitsfördernde Elemente, wie im FHN-Konzept enthalten. Als Quellen wurden Datenbanken und das Internet benutzt. Eine WHO-Studie zur Etablierung der FHN in der europäischen Region zeigte sehr große Unterschiede bei der Einführung des FHN-Konzeptes in den beteiligten Ländern. Vielfach scheinen grundsätzliche Probleme in der Umsetzung zu bestehen, wie zum Beispiel die Finanzierung und die Anwerbung von Pflegekräften. Dies bestätigte sich bei der Analyse von verfügbaren Projektberichten für Schottland, Deutschland und Dänemark. Zusätzlich wurden verfügbare Modelle und Konzepte aus den Ländern Großbritannien, Schweden, Finnland und Niederlande untersucht. Schweden, Finnland und Großbritannien haben gemeinsam, dass in den dortigen Gesundheitssystemen Pflegekräfte vielfach bereits Kompetenzen haben, die sie in verschiedenen Settings unter anderem im Bereich der Gesundheitsförderung und Prävention anwenden. Insgesamt wird deutlich, dass es Anhaltspunkte für Potenziale der Kompetenzen und Organisationsformen von Pflege gibt, die in Zukunft verstärkt für die Sicherung der gesundheitlichen Versorgung genutzt werden könnten und sollten.
