The diagnosis type-2-diabetes confronts people concerned with the need to modify their disease-related behaviours. Though it is not easy to cope with these modifications on an every day basis, long term guidance for people with diabetes is demanded through education, instruction and counselling. The current health care situation of people with type-2-diabetes indicates that the German health care system is not sufficiently prepared for the needs of patients with diabetes. Several international journals conclude that by the use of interprofessional cooperation with an active integration of nurse-led interventions in the field of education, instruction and counselling, improvements in the care of people with type-2-diabetes could be made available. A low-threshold intervention concept in combination with the concept of empowerment could fulfil the existing needs of people with type-2-diabetes. -- Keywords -- type-2-diabetes, health promotion and prevention, education, instruction, individualized counselling, low-threshold intervention concept, empowerment -- -- Die Diagnose "Typ-2-Diabetes" konfrontiert die betroffenen Menschen mit der Notwendigkeit einer Modifikation ihres gesundheitsbezogenen Lebensstils. Eine Integration veränderter Verhaltensweisen in den Lebensalltag ist jedoch nicht einfach zu bewältigen und erfordert eine langfristige Begleitung von Typ-2-Diabetikern durch Schulung, Anleitung und Beratung. Es kann allerdings davon ausgegangen werden, dass diesem Versorgungsbedarf kein entsprechendes Versorgungsangebot innerhalb der Primärversorgung gegenüber steht. Durch eine berufsgruppenübergreifende Betreuung unter aktivem Einbezug von Pflegenden in den Bereichen von Schulung, Anleitung und Beratung können Verbesserungspotentiale im Hinblick auf die aktuelle Versorgungssituation erschlossen werden - diesen Schluss legen zahlreiche internationale und einzelne nationale Studienergebnisse nahe.
